The main reason why headache gets worse when the sleep pattern is disrupted is a loss of the rhythmic balance of brain activity. During normal sleep the nervous system rests, vascular tone stabilises and the level of the neurochemical substances that respond to pain is regulated. When sleep is reduced or irregular, this balance is lost, and as a result a headache appears or an existing one becomes even stronger.
Several mechanisms explain this link:
1. Changes in neurotransmitters.
When sleep is reduced, the levels of substances such as serotonin, dopamine and melatonin change in the brain. A fall in serotonin in particular causes the blood vessels to widen and migraine attacks to be triggered.
2. Disturbance of vascular tone.
Lack of sleep can result in narrowing of the blood vessels in the brain or, on the contrary, in their excessive widening. These changes make the blood flow to the brain unstable and lead to the activation of pain receptors.
3. A rise in stress hormones.
Too little sleep raises the level of stress hormones such as cortisol and adrenaline. These hormones increase tension in the nervous system and tighten the muscles of the neck and head, which in turn causes tension-type headache.
4. Increased sensitivity of the brain to pain.
Sleep deprivation reduces tolerance to pain. Normally the systems in the brain that inhibit pain are active, but when sleep is disrupted these systems weaken and even ordinary stimuli are perceived as pain.
5. Activation of migraine mechanisms.
When the sleep rhythm is disrupted, a process known as cortical spreading depression takes place in the brain. This is an electrical wave that plays a part in the onset of migraine and is closely linked to changes in sleep.
Keeping a stable sleep pattern is one of the simplest but most effective ways of preventing headache. Going to bed and getting up at the same time every day, staying away from screens before sleep and not taking caffeine or heavy food in the evening all help to protect this balance.
In short, when the brain is left without sleep it becomes strained at the chemical, the vascular and the nerve level alike. And that creates ideal conditions for a headache to develop.
