Treatment of post-traumatic headaches

Post-traumatic headaches (PTH) usually develop in the period after a head injury, over several weeks or months; they may spread on both sides or on one side and often show tension-type or migraine-like features.

Treatment in the post-traumatic setting consists of several stages. In the first stage a detailed neurological assessment is carried out: the severity of the injury, its effect on brain function, and an assessment of other symptoms (for example difficulty concentrating, sleep problems, psychological stress).

For symptomatic treatment, painkillers (NSAIDs, paracetamol) and the symptomatic medications suitable for migraine may be used. Preventive treatment  may be planned with an antidepressant (amitriptyline) and anticonvulsants (topiramate, gabapentin).
