Treatment of headache begins with the right diagnosis.

Diagnosis and treatment

Clinical approach

Many patients living with headache get by on painkillers for years without receiving a precise diagnosis. Yet headache is not a single disease: the international classification describes hundreds of its forms, and each form has a different treatment strategy.

For this reason, the first step at the appointment is to establish the form of the pain. The history of the complaint, the character of the pain, its frequency, its duration and the factors that provoke it are examined in detail. The diagnosis is made according to the classification of the International Headache Society.

Once the diagnosis has been established, treatment is planned individually for each patient. In many cases medication is enough; in more stubborn cases nerve blocks and injection treatment are used.

Procedures used

  • Botulinum toxin (Botox) injections
  • Greater occipital nerve (GON) block
  • Lesser occipital nerve (LON) block
  • Supraorbital nerve (SON) block
  • Other peripheral nerve blocks
  • Preventive anti-CGRP injections

Frequently asked questions

When should you see a doctor about a headache?

If the pain recurs several times a month, if ordinary painkillers do not work, or if you need to take them several times a week, the examination should not be delayed. Some signs call for urgent attention: very severe pain that starts within seconds, pain accompanied by fever or neck stiffness, disturbances of vision, speech or movement, a change in consciousness, pain that appears after a head injury, a new type of pain starting for the first time after the age of 50.

How is the diagnosis made?

The diagnosis of headache is made mainly through the interview and the neurological examination. When the pain began, which part of the head it affects, how long it lasts, in which situations it worsens and how it responds to medication are all examined. Additional investigations are arranged when necessary. Keeping a headache diary helps to refine the diagnosis.

Which procedures are used?

Botulinum toxin injections, greater and lesser occipital nerve blocks, supraorbital nerve block, other peripheral nerve blocks, and preventive anti-CGRP injections. The procedures are carried out by the doctor.

Are children and adolescents seen?

Yes. Children, adolescents and adult patients are seen. Headache in children and adolescents is one of Dr Sarkhan Amirguliyev's areas of research; his article on this subject has been published in the National Journal of Neurology.
